Open up the toolset, then do file-new-morph. Choose the base(elf,human,dwarf, and female or male) then edit it to your liking. When you're done, save it, so you can edit it later if you want to. Then go to tools, export, and then post to local. Open up a save, expand the player character line, then appearance. Open up the .mor of the morph you just made, it's located in documents/bioware/dragon age/packages/override/toolsetexport. Type the name of it under the morph line under appearance, then save the save file.

In addition to that, if, say, you're happy with your character BUT the hair/beard color/model (say, you're like me who spent 1 hour designing a face only to find out the hair model clips with the back of the head when looking sideways), you could use the toolset to find the name of the color/model you have and the one you want, and use a hex-editing program, set it to search unicode and search for the string on the save file (and replace it).

What do you men by "open up a save"? I'm stuck on that step.
