Okay... sounds like a fun one.

Firstly, the game auto-saves on fast travel by default. Loose that if you haven't already. Auto-save is buggy as hell, causing regular CTDs and eventually corrupted saves. Thats not mods, thats a problem with the original autosave feature. Quick save seems to be free of these issues, but if you are going to use it, delete the quick saves and make full saves to replace them regularly. I generally use quick saves for no longer than an hour or two, or if I get my hands on something really cool or complete a quest, then make a full save in a new slot and delete the quick saves.

Next, look see how long you can stay ingame for. Unfortunately, due to Beth rushing it out for the X360 Launch date, Oblivion was never all that stable to begin with, so once we started modding it, it was bound to get worse. However, if you're like me you wouldn't still be playing the game if it wasn't modded. My game tends to crash maybe once or twice a night, and for how I've got it set up, I consider that perfectly acceptable.

So the next thing to look at is probably how frequently you crash. How long is it before you crash? Just randomly, but regularly enough to make the game unplayable? Or does it seem to be at around the same length of time (for example everytime after spending say, 20 minutes outside). Reason I ask is that Oblivion can be quite the graphical resource hog, particularly in that immense wilderness, and even more so when modded. It could be a sign that your graphics card is on its way to silicon heaven if the crashes are frequent and usually after roughly the same length of time. PLLLEEEEAAAAASSSEEE don't run out and buy a new card just yet on my say so... lets see if we can get to the bottom of any other problem that might be causing this first. Just figured I'd give you fair warning.

For troubleshooting purposes, deactivate Streamline & DR. These two make a whole heckuva lotta changes to a whole heckuva lotta things, making troubleshooting an individual issue near impossible with them active. In fact, unless you're on a low-end system, you might want to get rid of streamline altogether, unless you've noticed big changes. Personally, on mid range systems, the five or six extra FPS I noticed it bought me in fights outside weren't worth the trade-offs in general performance and quality, and on high end systems it like as not won't buy you anything.

Next, start looking at your wilderness affecting mods... of which you appear to have quite a few. Begin by taking out any that have altered the water. Well, thats how I'd start, as frankly I don't spend a right lot of time in it, and there's a heckuva lot of things that can be changed beneath the surface that can severely affect performance that you generally wouldn't notice, let alone care about, unless you were the Little Mermaid incarnate. This is particularly significant if your game seems to slow down or crash more often around the heavy water areas, like Lake Rumare or the coastal regions.

Next, I'd start looking at maybe Enhanced Weather. I know, its a cracking mod that fixes something Beth should have included in the game in the first place, but it can slow down your game, and anything that can slow it down can ultimately crash it.

Remember, you don't need to deactivate a whole bunch of mods at once, and if you do, you're likely gonna struggle to figure out just what was causing the problems. Go for one or two at a time and playtest for a while. One crash might just be a fluke, so never take the first one as fatal either.

If none of this seems to help, holler, and I'm sure someone else will have some advice to offer if I can't help further.

Good Luck.

Jenrai
