If your alchemy skill isn't high enough, you won't be able to discern all of the effects.

 

As for hidden effects, do you mean Scripted Effects? They're practically so rare I doubt you really need them. Only place I've seen them are on Count Cirion's Helmet and the Boots of the Crusader.

That's because your alchemy skill is too low to determine it. Get better at alchemy. At mastery, you'll be able to see all effects.

For example I got a ring from a NPC, Ancotar's_Ring_of_Protection. Ancontar told me the ring would protect me against the side effects of the Reverse Invisibility Scroll. But I don't even know if that's true, the ring doesn't seem to have any enchantment. I wonder if it's worth keeping =/

Hmm.. I think that if it's quest related, you might as well just follow the NPC's instructions concerning it. Like the Staff of the Everscamp. You might be able to see if it has something called a 'scripted effect' under its effects, but if the quest itself just checks to see if you've got it equipped, you won't see it.
