This thread is for hard core mode discussion. I see companions references appearing along with information that could spoil game surprises. The bugs mentioned do not imply hard core problem solving.

Hard core bugs should be to adjust realism. Like: the rest required to regain body strength when water is for flexibility, blood flow for delivery of food, and carrying out waste. Water is not a restorative and should not be considered as a solution to restoring strength. The repair and restoration that would give back strength is done by the red blood cells carrying food and oxygen to the internal organs and muscles, etc..

If we are going to have hard core status in the game we should also have reasonable understanding for the restorative powers of the various purposes of food, water, and sleep in the game process.

Companions are being mentioned because in hardcore mode they die for good.

One other thing.

A loss of strength means a loss of muscle mass and the game does not take that into account. The player would have to go back to the Doc in Goodsprings to spend a few weeks rebuilding their muscle mass to be able to carry the load they were accustomed to, too.

Without food, water, exercise and rest muscle is not rebuilt readily. After losing ten points of strength in the game in hard core mode it would have more validity if you had to consider spending six weeks, as a minimum, in recovery; rather then a few seconds after a few ounces of water is drank and the fatty reserve in the body begins to restore stamina as it is burned.

Strength still takes muscle and once muscle is gone a few minutes later all your companions are going to be needed just to save you from attacks while you drag yourself back to Goodsprings or the doctors hospitable tent nearest you.

Sounds like the seeds of a new mod :)
