How about, if someone hasn't made any obvious activity for at least a year on an account which is desired by someone else, send the current account holder a PM (or email) asking them to confirm that they stil use the account, and if they don't reply in a month or so, then release it. I think that's a fair and just time scale. Also, as for name changing, how about charging a small administration fee for it. Something inconsequential like £1.
