Vortaka
For the driver bug, have you tried running Fallout 4 in windowed mode? I cannot run it myself in full screen, always crash at launch, but windowed/bordeless mode works fine... Vortaka

kintaro12
Drop settings of the game, if not getting better, prepare for a new GPU

Grasmann
Maybe you should check the temperature of the gpu and cpu when playing.Might be an overheating problem. Could be all kinds of other problems too, though. To me it looks like either a problem related to shaders or as I said maybe overheating or something.
