I mean, other than Preston getting all butt-hurt if you don't. Does it open any quests, or do you meet some of them topside afterwards, or something? FOR SCIENCE! ;) Actually, it does affect one thing, assuming the Railroad still exists: 1. If you didn't evacuate, the Railroad will become hostile, because, OMG, you killed every synth down there. 2. If you did evacuate, now the Railroad safehouses are full of synths, and the Commonwealth is full of people who would rather kill every single synth. Which opens a new series of repeatable radiant side-quests to clear the way to move those. Interestingly, option 2 also gets you the unique chane to hear Desdemona that the humans in the Commonwealth have always been a bigger enemy than the Institute. Uh, seriously? I can understand wanting to free the synths, and I can understand not having the resources or manpowers to help the humans too, but at the point where the humans are the enemy... something is wrong with her head.
