A couple of points...

(1) Rain. Due to a slight hiccup, buildings don't block rain. If you stand under an arch, you still get wet. If the insides of buildings were attached to the doors, then in all probability you'd get leaky houses.

(2) The TARDIS effect - "It's bigger on the inside" - some houses just don't fit inside their buildings, or don't line up with the doorways. The Tiber Septim Hotel is a culprit, for instance.

If somebody would finish/evolve Reneer's Rain Mod which allows buildings to have own rain colission: http://tesnexus.com/downloads/file.php?id=29508

And you could set the scale of interior and exterior the same, maybe even add them together creating one model (?)

But there is one thing you couldn't do:

Dungeons, caves, ayliied ruins: As they need to be a separate cell.

Another issue is performance. Even with one exterior house, you can get drastic framerate drop near that house, regardless of your computer spec. As you see, this game has AI packages that CPU should process, and the game engine doesn't like many actors in one cell(if there're too many, about 20~30 or over, actors are present in one cell, you can get performance issue and/or brain-dead actors). And GPU should render 'all' the models.(Check MTAEVWD, and find out why it is not recommended for actual playing)
