It's a bit more fantasy and TES than fallout but I think a new vampire faction could be cool. And sorry for the massive wall of text that's coming, it's a fairly large idea.

 

They could have been created after the bombs fell, with groups of survivors trapped in dark and highly irradiated areas, being forced into cannibalism in order to survive. They would have a few things in common with ghouls, such as incredibly long lives and a chance to turn feral. But the longer they live the more powerful they become, so 'prewar' vampires (inverted commas because they weren't vampires prewar) would be incredibly powerful leaders and have a nest of younger vampires following them.

 

The vampires would have a naturally high damage and rad resistance, with enhanced AP, strength, agility and melee/unarmed damage, and a natural cannibal ability. But when outside in sunlight will take damage similar to radiation damage with a new bar reducing health. This damage could be removed using a blood pack or feeding on a sleeping npc or corpse. There could be a 'stages of vampirism' system like in skyrim, whereupon not feeding for several day will result in a new more feral stage with increased sunlight damage but stronger melee and unarmed attacks. The natural cannibal effect could also be used to convert settlers into vampires.

 

Being a vampire would affect a few perks. The solar powered perk would lose its current benefits and instead offer increasing resistance to sunlight damage. The night person perk would also add strength and agility points along those it already gives. The cannibal perk would remove increasing amounts of sunlight damage and give a temporary boost to sunlight damage resistance.

 

The vampire faction could be worked into the main quest line. Synths would be poisonous to vampires so they would be natural enemies with both the institute and railroad. They would want to convert some settlers and use others as prey, as such would be enemies with the minute men. And given the BoS's hatred of anything non human they would also be natural enemies.

 

The quest lines could be made a bit different to the vanilla 'just destroy everyone who's not us' faction quest lines. Yes they would want to stop the institute and destroy all the synths, but would view the actual institute as an underground utopia free of the damaging sun, and would want to capture it and convert the scientists instead of destroying it. Instead of destroying the minute men and BoS, there could be a campaign to usurp the leadership of one/both factions and replace it with vampires giving them a private militia. Finally the vampires would just want to destroy the railroad for their views on synths, there could be an option to capture PAM and use her to track down and kill relocated synths in a series of repeatable radiant quests. Or you could just destroy everyone who's not you if you're the blood thirsty type.

There could also be a quest line to reject the vampires, leading to trying to convince the other factions of their existence and then eradicating them. This could also be used to try and broker alliances between the minute men and railroad or BoS to fight the vampires.

 

Both sets of quests could be started by a settler coming and telling you of some strange things s/he saw at a location sending you to investigate. Once there you'd fight you're way through the nest until meeting its prewar leader. S/He would be able to tell you were also prewar (smell less irradiated maybe?) and suggest with your 'pure' DNA you could become a far stronger vampire than anyone else and lead them to conquer the commonwealth. At this point you'd either become a vampire and start their quests or attack and kill it and start the other factions.

 

There are also a lot of side things possible with the settlement side of things. Vampire settlers, obviously, who would sleep during the day and do whatever settlers do at night. New objects like coffin beds, walls that aren't full of holes to keep rooms light free. Pens to keep the non converted 'cattle' settlers from running away. You could even use some of the existing hospital models to create blood donating equipment which would function like water purifiers but add blood packs instead of water and require a cattle settler to work.

A new attack style feature could have the cattle settlers arming themselves and trying to take back the settlement or escape and you'd have to try and use non lethal force to recapture them, or just kill them and get more.

You could even set up an extra happiness system (hopefully better than Bethesda's) relying on enough blood produced per day per vampire (with live feeding adding a happiness boost over blood packs), the vampire:cattle settler ratio, cattle settlers having 1 food and 1 water a day to keep them healthy, enough light free beds for vampires etc.

 

And in case all that's not enough, you could even have different vampire bloodlines stemming from different locations the original vampires got trapped and created (similar to the vampire bloodlines in morrowind). This could add a whole different sub story with multiple places to become a vampire, differing strengths and weakness of different bloodlines, even a super strong 'boss' clan living deep in the glowing sea. It would add an extra series of quests where you could destroy all the other bloodlines and conquer the commonwealth in the name of your bloodline, unite the bloodlines peacefully or conquer them one by one and rule over them as the dominant bloodline, create a complex web of alliances and enemies and use your charisma to stay in power. Or you could become a vampire and start your own bloodline using settlers and conquer, destroy, form alliances or just ignore all the others.

 

If you've read this far, thank you for hearing me out. I am aware from creating a few skyrim mods just how large this request/suggestion is and is firstly well beyond my abilities and free time, and secondly will probably take a team quite a while to do well. So if someone feels like taking it on you have my sincerest thanks and a wish of good luck with the whole thing.
