I'm not sure this is actually possible for an NPC in game.

If you use EFF http://www.nexusmods.com/skyrim/mods/12933/? and select tattoos under MCM window options AND you have a weight slider along with a proper skeleton, you can alter NPC's. However just remember your armor will alter your shape unless you plan on the NPC running around nekkid.
