RoccTrap
I recently bought an SSD for my computer. I backed up all my data and did a clean install. After I reinstalled Fallout 4 and the mods, all my mods have stopped working. As an example, the ponytail options are in the character creator but the hair will not load. I looked around to try and troubleshoot the issues. Fallout 4 and NMM are installed on my old HDD.
BlahBlahDEEBlahBlah
Reinstalling Fallout might have replaced your old .ini file, maybe? If you're using Nexus Mod Manager, there still needs to be some necessary edits to your fallout4.ini (if I remember the name right) file to get loose mod files to be read/seen. RoccTrap
I did that. Not only that, but I backed up my .ini just in case this happened. Copied those over in case I missed something and same issue.

Zeppelin1996
I had the same issue when I installed my SSD. The problem turned out to be Microsoft OneDrive. The game was reading the .ini files from OneDrive. After I uninstalled OneDrive, everything worked fine.
