dubiousintent
Please see the 'Issue: Ambient music skips/jitters/hiccups/pops/stutters' entry under the 'Solutions to Performance problems' section in the wiki "Fallout NV Mod Conflict Troubleshooting" guide. -Dubious-

dubiousintent
The "codec" package is installed like any other Windows program. The Vorbis OGG Library is a set of DLL files which you have to manually install/copy into the game root folder (where the "FalloutNV.exe" file is found). Mod Managers don't recognize or place them properly. They will need to overwrite the existing files. -Dubious-
JimboUK
There is something that I've noticed, if you push the game too hard then the music can start to stutter, I was running it at 4K maxed with an army of companions in high res gear, this on top of the texture packs and everything else, without ENB Boost the whole lot would have fallen apart. When I swapped out my way too big 4K display for my old 2K one the problem went away.
dubiousintent
@jim_uk: Thanks for that anecdote. Added it (unattributed; will change that if you wish) to the conclusion of the wiki "Display resolution versus Image Size" article as an example. -Dubious-

dubiousintent
You said it was only happening when you start the game. If defragging the disk drive along with the codec and DLL replacements didn't resolve the problem, seems like you are left with using Process Explorer to track down which app or service is grabbing CPU cycles. -Dubious-
eren2707
No it happens 5 minutes when i start the game, dubiousintent
dubiousintent
Please provide ALL the information requested in the wiki "How to ask for help" article. Don't forget to identify things that do not appear in the "load order", such as NVSE and it's related plugins, texture replacements (specifically their larger image sizes: 1024x1024, etc.), and "post-processors" like ENB or SweetFX. It may be that you are pushing the limits of what your system can handle. -Dubious-
