You are going to want NVAC (New Vegas Anti-Crash) and probably YUP (yuki-whatever-his-name-is unofficial patch, search for New Vegas unofficial patch to find it).

 

FNV is an old 32 bit game. By default, it only can use 2 GB of memory. It doesn't matter if your PC has 48 GB of ram installed. The game will only use 2 GB of it at most. There is a 4 GB patch available which will double the amount of memory the game will use. If you are using mods, you will want this patch.

 

At some point, pretty much no matter what you do, you will find the game getting slower and slower, and eventually the game will probably just crash on you. This is because the game leaks memory. Having the 4 GB patch installed means that the game has twice as much memory to lose before it crashes, so you'll go longer between crashes, but at some point the game is just going to crash on you. Save often, and if you notice the game slowing down, that's a good time to save and restart your game (exit all the way out of the game and run it again - don't just exit to the main menu).

 

The main memory leaker appears to be the game's texture caching system. Using high resolution texture mods make the game look prettier, but the extra load from the higher resolution graphics makes the game crash sooner. Unfortunately this means that you have to make a choice between a pretty game or a stable game. With high resolution graphics mods and ENBs and such, you can make the game look just as good as FO4 or many other newer games. Unfortunately, your long term stability will suffer. I personally prefer stability over pretty textures, so I avoid graphics mods, and I went through a bit of trial and error with my system to see how high I could crank up the graphics settings while keeping the game stable. A lot of people think that the game is going to crash no matter what you do, but I have made my game very stable. I won't say that it never crashes, but crashes for me are very rare. Some people don't like old clunky graphics though. For many people, high resolution texture mods and appearance overhauls like FCO (Fallout Character Overhaul) make the game look so much better that they are willing to suffer through the occasional crash to have a game with far better graphics.

 

Another thing I personally do is I save an original unmodded copy of the game (just copy the game folders somewhere). Then I add mods and test them out, and once I have a stable combination of mods I like, I copy that version somewhere as well. This way, no matter what happens, I can easily get back to a configuration that I like. Most PCs have gobs of disk space these days, so all of those copies aren't a big deal. This can save you a ton of work if you have a mod that you end up not liking, but then the mod doesn't uninstall cleanly and trashes your game. Rather than spending hours sorting through the files and trying to figure out what got messed up, you can quickly copy your old stable version back to your game folder and get a working game back.

 

Once you have a stable game, where you want to go after that is entirely up to you. Some people prefer realism mods. Others prefer mods like my little pony or whatever (not my thing, but hey, I'm not judging). The point is, FNV is an old game with a lot of mods available. Chances are you can find some mods that will shape the game into what you want it to be, whatever that is. There's a list of the most popular mods on the Nexus, so you can look through that and see what other people think are the best mods, but really the best thing to do is just look through mods and see what appeals to your own preferences.
