Show Looks Menu changes do not stick to the NPC

I am running the latest version of the Show Looks Menu and I use presets downloaded from Nexus. My last few changes to some NPCs have looked fine when I did them but when I return to the game later, the character's face is distorted into a large eyed, pinched faced visage that barely looks like how I left it. Two recent examples are Myra the general trader in Diamond City and to Polly the meat trader in Diamond City. I have attached a picture of Myra but Polly's distortions are the same type. It should be noted that the hair changes made in SLM do stick to the NPC.

What am I doing wrong in my use of SLM?
