Strange crash when travelling?

I've recenty been getting a strange crash type bug in Skyrim that's really getting on my nerves.

 

For some reason, when I travel somewhere via carriage or some other doors then the game stops responding. You get a loading screen then everything goes black, but you can still hear the music and sound effects of the region that was meant to load.

 

Any help?
