Companions "leash" during combat

So they won't charge ahead to a deathclaw nest with a police baton, or ruin your cover at the very second a fiend 100 m away will get a vague sense of suspicion, but stay within 2-3 m radius.

 

Also a good bonus would be that companions would try to stay by the sides or behind player, at least when using ranged weapons.

Have you tried "JIP Companions Command and Control (JIP CC&C)"? It has "true passive" mode (won't attack anything until you do) and you can have them individually follow at either "close" or "distant" range. Also look into "Move Followers Aside" which keeps them from moving within melee range of you.
