HamoDragonborn
I love the roleplay mods that immerse you into the game world a bit further. Lately I've been really enjoying the grow a hair mod (check it out if you haven't already, it's great) which makes your hair grow dynamically as you progress through your game, integrating the hairstyle mechanics seamlessly into your gameplay. That got me thinking, what if we had similar system to bring scars into the mix? And how about if we could mend the unimmersive aspect of dying and reloading to your last save at the same time. And the way I propose we do that is to make a system in which instead of dying, you get transported back into your base settlement, but bearing a random scar as a consequence of your injuries. I think this system would add to the immersion into progression of your character from suburbian father to wasteland wanderer with the battlescars to prove it. What do you guys think? Would that be a difficult thing to code?
PrincessNiya
I've been looking for something like this. Also, if modders could incorporate the various blemishes and sickly appearance for things such as radiation poisoning / addiciton. There are a lot of cosmetic effects that go unused. That's a great idea, it would be a great thing to see. It's details like that that make the world seem so much more real.
pra
No idea about alternative death, but in general, yes. Get reduced to 10% or so of your health, you get a scar. Get sick, you get a skin rash decal and/or get pale?Sunburn is technically radiation burning, so maybe use that for rad poisoning? You could probably go even further and increase the dirt decal level over time, too. Until you enter a body of water or something?

And apparently Horizon already has the alternative death system in place, that functions basically the same as I've proposed. The screen fades out and you wake up in the nearest settlement, with the message that your companion pulled you out to safety, and a certain penalty in caps that took to cure you. So basically, all the pieces of the puzzle are there, they just have to be put together. :D
