Ulfric's gag from Skyrim to Fallout 4

Would it be possible to create a craftable version of Ulfric's gag from Skyrim in Fallout 4. Which you can wear and equip NPC's with.

 

Would it also be possible to make it so when equipped by player or NPC, their speech becomes muffled, like in real life, then return to normal when unequipped

 

I play on xbox 1
