Guys, great news. The brilliant people over at CP77 Modding tools discord figured it out.

1) Download and install https://wiki.cybermods.net/cyber-engine-tweaks/setting-up/installing

2) boot up your save file and bring up the console (press the '~' key) https://wiki.cybermods.net/cyber-engine-tweaks/console/usage

3) paste this command "Game.SetDebugFact("sq032_johnny_friend", 1)" and hit enter
Make sure you do this before starting the mission. Your 'You only live once' percent should increase by 10% (eg:- if it's 40->50, if it's 60 -> 70). That's it! Secret ending should be enabled.

In case someone searches for this - command also works even if you enter it right before sitting in a chair, don't have to restart or enter it before starting the quest.

Also, all the info about conditions doesn't appear to be correct. In my case I didn't know about those conditions or secret ending, but I still chose the same exact things in "chippin in" (carve initials, guy who saved me), also I had 60% (no idea what I chose to upset him), and secret ending never triggered. This is perhaps the stupidest and most random thing I ever encountered in any game, just need to guess the developers mindset or stumble upon the ending by yourself by accident.

Yep, I have exactly the same thing where I have 60% with Johnny and I don't know where I made a mistake.

But because of it, I'm not getting the secret ending.

Edit: never mind it worked for me!

Couldn't agree more. Another option might adding a marker of some sort to the dialog options which are more "optimal", and thus unlock the harder to reach story progression points.
