I've encountered that too. The gem isn't with Shar's statue at the end of the room. I've tried reloading, which fixed an earlier bug with a follower not following me, but to no avail. EDIT: I am pounding my head on my desk because I had not interacted with the shrine to Shar at the beginning of the Trial. You must shed blood before the gem materializes.
