How do modders add items to game nowadays?

You can set them in specific places to be found.

You can add them to one or more leveled lists directly and indicate to users that they should create a bashed patch for the leveled list.

You can add them to one or more leveled lists via script to avoid needing to make a bashed patch.

I am sure there are other ways as well.

I wouldn't say creating an NPC merchant with their own hidden chest is easier than adding items to an existing leveled list used by the existing merchants. Don't be afraid to edit the leveled lists out of fear of conflict. Not every mod that edits leveled lists will conflict. And for those that do, the end user can create a bashed patch. If anything, I'd say the custom NPC merchant might have less chance for conflict. Then again, a mod might change the area where you put the NPC and they get stuck, lost or worse.
