I got the puzzle box ane gave it to the guy at the fighter's guild but I don't seem to have the report for Caius (sp?) When I talk to Caius, he still tells me to see the guy at the fighter's guild.

Try talking to the guy about all the topics. You could also try reverting to a saved game before you took on the quest. If those don't work use the editor to give yourself a copy of the notes. If all else failes reinstall the game because It's probably an error. That won't be too bad because you probably aren't very far anyways.

Have you got both expansions? If so there is a bug in it that effects the main quest, but that they fixed in the patch. You could try that before restarting
