To use VSync without clamping FPS, triple buffering is a need, yet it's something tricky to do with DirectX and need a third part application to force it (Nvidia drivers have an option to activate it but works only with OGL). The primary advice is disabling Vsync at all (most applications will do well) directly in the video card drivers and in applications that allows it. The reason is plain simple, if the board can't do 60 frames it will clamp at 30 and if it can't get 40 so it will clamp at 20... that already linked above Tweakguide site is a must see for anyone having performance issues. KISS MEH Idk there's so much to running this game, it's not only a nighmare for the new people getting into itbut also the people that are already pretty deep into it. LOLZ ZOMG that's so silly You can't tell the difference with the human eye between 30FPS an 60FPS It's a benchmarking pee pee contest Also like many other things, unchecking Vsync via the fallout launcher doesn't turn it off. Nice they put the button there though huh :D it's so shiny click it mmmmmmmm The whole point of tripple buffering is to make Vsync work better sorry I'm totally lost Anyway that was just something I had not tried You can disable vsync via the falloutlauncher and then editing the notepad.ini to do so from what the tweek guide said, then maybe force the VSync with the grapics cardvia override 3D application setting, which might work better than fallout's I mean the AA an AF in the grapics card worked better than Fallout's Stands to reason. It does list Force on vsync in the interface advanced part.
