Jokerine
It would be possible (and maybe easier to avoid messing with NPCs and DLCs) to simply place two boats that would be activators at both cities. Then the player would activate 'em, they'd take away some gold or whatnot, and teleport the player to the desired location. The scripts would be simple and one NPC could just be placed there for flavor.

In fact, going this route, one could place boats on each side of every obstacle. They choose to travel to Riften or Ivarstead but only travel to the next portage spot and thus need to walk themselves over to the other boat to continue the journey. Partial fast travel with somewhat realistic stopping due to hazards in the waterway. Wouldn't need to have a message box asking where to go if approached at some point in the middle as each boat can only go one direction due to the hazard/blockage. Hallgarth
The easiest thing to do is probably to do what they did with the Dawnguard boat as stated (which I believe is just a door in the boat) and it teleports you to a boat outside of Ivarstead. At the end of the day, all the obstacles don't really matter, as the Carts (if you've used one of those scenic carriage mods) take you through all sorts of danger.
