M48A5
First, install the game, just Fallout 3, no DLCs. See this topic: http://forums.bethsoft.com/topic/1465357-installing-fallout-3-with-windows-78-and-xp/ Get the 1.7 update from Bethsoft Nexus. http://fallout.bethsoft.com/eng/downloads/updates.html download and install it. Get the Games for Windows Live client: http://www.xbox.com/en-US/live/pc/DownloadClient download and install it. Start and run the fallout 3 installer and watch the intro until you come to the page with the options to "Continue", "Load" , "Start New" and choose "Quit". Shut down your computer and restart in "SAFE" mode. Get the second disc, the one with the DLC content and install it. Do a computer restart and launch the game. The game will go to a page where it will detect your video card and you are able to check your game settings. Download and install the mod manager of your choice. Fallout Mod Manager is better for Fallout 3 than Nexus Mod Manager. If you only have Fallout 3 and not Fallout: New Vegas, use Old FOMM http://www.nexusmods.com/fallout3/mods/640/? Download and install FOSE: http://www.fose.silverlock.org/ When you open FOMM, you should see a button for "Settings". Click on it and in the "General" tab, uncheck "Check for new FOMod versions".
