Females still wear underwear as do guys, it just looks a bit different. BAB is female body replacer, so it won't affect males at all(if you can avoid infamous Khajiit body texture problem). For males, Robert's male body replacer has several options, including slender/scrawny body type. However, I don't think I can find clothing replacer for slim male body....
