In previous games adding a new perk to the interface just ment adding a new line in the Pipboy. But with the F4 interface I'm not sure how one would fit a new perk in.

 

You must add a new PERK record with all the required sub-records (EDID, FULL, DESC, Conditions, DATA, SNAM ,NNAM, Effects)

 

Then you can add a new pip boy animation for it by copying and editing one of the SWF's inside "Fallout4 - Interface.ba2" in this directory "\Interface\Components\VaultBoys\Perks"

My guess is that'll look like Skyrim's skill/perk setup, but in a straight line. We'll have to wait for the GECK to know for sure.
