My understanding of the simulated Witcher 2 save is that if you didn't play the Witcher 2, you will get chances later in the game to answer some questions about the events of that game, thus "simulating" the Witcher 2 experience. By saying no, a preset world state is made for you.

NG+ means "New Game Plus", which means after you finish the campaign the first time you're allowed to play the game all over again, but without losing your level, stats, gear, etc. The game will naturally make itself more challenging to compensate for your higher level character though if you do a NG+. I think once you reach a certain point very early in the game on a NG+ save you will be able to access your old gear, you will not just start the game with everything you had before.

I rushed into the game since this was the first time I have played it, so I ignored both of those options. Thank you for explaining it.
