The "witch" that ran off with Cloudcleaver can be found standing on the back path to Ald-Ruhn. No witch is associated with the NPC that has witchwither -- just cure his disease and he will give you a fashionable hat. I was going to kill the Witch, then Give the Nord his axe, then kill the Nord and get it back..... But I decided against it. And I don't mind the open quest.
