If it's the same as Fallout and Oblivion you make a custom race and have that race use that body.

Yes, create separate folders for the meshes and textures for your race. And be prepared to do a bit of scripting if you want to go vamp or werewolf.

Will it work in offline mode?

If it's anything like mine, no. For me, the CK has always insisted on having an active online connection. Even before uploading anything to Steam. Probably because it needs the cloud support to be on at all times or it won't even load.
