It's entirely subjective, and I usually end up disagreeing with the results.Don't get me wrong, I'm not saying my mod should be the winner or anything like that. All I do is make signs and posters. There are some mods that have added some really great stuff to the game.The most popular ones, though, tend to add something that lets you cheat (gives you uberstats, lets you stack 8x as much defense as intended, gives you autofire mininukes, etc) or adds nekkid bewbs. And that's not what I do, or what I use. For those who do, enjoy. Modding is to add what you want in your game. To each their own, and it's great that the things that make your game more enjoyable for you are there. But what I like may well be different from what you like, so my "best" mod choice may be vastly different from yours. Personally, if I had to choose a "best", it would be based on what added the most to my game for my enjoyment. And at this point that would be something like Snap'n'Build, or Homemaker, or OCDecorator, that made my settlements interesting instead of cookie-cutter dumps.Eventually, it will probably be a content addition mod that adds a huge amount to the game like has been done in previous Fallout games, but nobody's put one out of that scope yet that I'm aware of.
