The face and stats can be imported to a saved game or a plugin using Wrye Bash as shown in this tutorial wiki for the savedgame, the rest is a matter of simply following the CM Partners Tutorial that come with that mod.

 

I will have to do the wiki on importing it into a plugin but you can follow the steps I've outlined in my response on this thread in the meantime.
